Diese Episode beleuchtet die messbare Resterkrankung, MRD, bei der AML. Prof. Dr. Felicitas Thol und PD Dr. Amin Turki erklären, warum die MRD-Diagnostik bei AML besonders anspruchsvoll ist: Die Erkrankung ist molekular heterogen, die verfügbaren Marker unterscheiden sich je nach Subgruppe und die Methodik muss sorgfältig gewählt werden.
Besprochen werden die drei zentralen diagnostischen Säulen: Durchflusszytometrie, RT-PCR bei geeigneten molekularen Markern wie NPM1 oder bestimmten Fusionen, sowie zunehmend NGS-basierte Verfahren. Die Experten erläutern, warum Material, Zeitpunkt und Laborerfahrung entscheidend sind, weshalb Knochenmark häufig sensitiver ist als peripheres Blut und wann MRD-Messungen besonders relevant werden – insbesondere nach Induktionstherapie und am Therapieende. Deutlich wird: MRD kann heute helfen, Rückfallrisiken früher zu erkennen und Therapieentscheidungen, etwa zur Transplantation oder Intensivierung, präziser zu steuern.

Wichtige Publikation: Jongen-Lavrencic et al., New England Journal of Medicine 2018. Die Studie zeigte, dass molekulare MRD in kompletter Remission unabhängig prognostisch für Rezidiv und Überleben ist.
In dieser Folge sind die führenden Experten Prof. Dr. Felicitas Thol und PD Dr. Dr. Amin Turki bei Moderator Dr. Friedrich Overkamp zu Gast. Im Mittelpunkt steht zunächst die Supportivtherapie – als zentrale Voraussetzung dafür, dass AML-Therapien sicher durchgeführt werden können. Diskutiert werden antiinfektive Prophylaxe, Pilzprophylaxe mit Azolen, antibakterielle Strategien bei langanhaltender Neutropenie, Antiemese, Schmerztherapie, G-CSF, Ernährung, Physiotherapie und psychoonkologische Unterstützung.
Im zweiten Teil geht es um die Therapie unfitter AML-Patienten. Die Experten ordnen ein, warum niedrig dosiertes Cytarabin oder hypomethylierende Substanzen als Monotherapie heute nur noch eine begrenzte Rolle spielen und weshalb Kombinationen wie HMA plus Venetoclax zum Standard geworden sind. Für Patienten mit IDH1-Mutation wird die Kombination aus Azacitidin und Ivosidenib als wichtige Option besprochen. Dabei geht es auch um praktische Aspekte wie Differenzierungssyndrom, Komedikation und die Möglichkeit, dass sich einzelne Patienten unter wirksamer Therapie so stabilisieren, dass später doch eine Transplantation infrage kommen kann.
